If you are ready to begin, please contact me and we can schedule our initial appointment.
Upon scheduling, you will receive a link to a client portal where you will enter your information. Forms include demographic information, insurance, and a general questionnaire. Completing the requested information prior to the session will help me understand how I can help you. If you are unable to access the information for any reason, please let me know.
I am in network with BlueCross BlueShield PPO & Blue Choice
If I am out-of-network for your insurance I can provide you with a monthly superbill/receipt upon request. Contact your insurance company if you have any questions regarding your insurance company's out-of-network benefits. Any reimbursement from your out-of-network insurance company will be sent to you.
Payment for all out-of-network clients is due at the time of service.
I accept cash, check and all major credit cards as forms of payment.
If you are unable to attend a session, please contact me as soon as possible. You may be charged a cancellation fee for sessions canceled less than 24 hours from scheduled appointment.
